POST-COURSE INFORMATION

TO REPORT YOUR CME CREDITS TO AAFP
1.  Go to www.aafp.org and log-in to your account with your AAFP ID# and password.
2.  You should automatically be redirected to My Account
3.  On the sidebar click on CME Proof of Participation
4.  This takes you to the My CME page.  Select Report my CME from the sidebar.

FOR ALSO PROVIDER COURSE PARTICIPANTS
5.  Under Course, click on “I attended a life support course”.
6.  A pop-up box will appear.  Enter the start date: 11/12/2009 and Course type: ALSO. Select "Provider" from the drop down menu
7.  This will bring up a list of ALSO courses; mark the checkbox next to:
     ALSO Provider Course
     Washington AFP
     Cle Elum, WA UNITED STATES
8.  Enter the total number of prescribed credits that you attended (26.75 max)

*Additional Note: AAFP Certificates of Completion and wallet certification cards have been mailed to your registration address.

FOR ANNUAL SCIENTIFIC ASSEMBLY PARTICIPANTS
5.  Under Course, click on “I attended a live CME activity”.
6.  A pop-up box will appear.  Enter the start date: 11/14/2009 and the state: WA
7.  This will bring up a list of CME courses; mark the checkbox next to:
     60th Annual Scientific Assembly
     Washington AFP
     Cle Elum, WA UNITED STATES
8.  Enter the total number of prescribed credits that you attended (download CME worksheet to calculate credits. 21.00 max)

How do I report my American Board of Family Medicine (ABFM) Self Assessment Modules (SAMs)?
If at the WAFP event you completed the knowledge assessment portion of a SAM, you will first need to go online to the ABFM website to complete the online Clinical Simulation portion of the module (this portion must be completed within 12 months of when you started the module).

When you complete the self assessment modules on the ABFM’s web site, the CME will automatically be reported to the Academy if you have given the ABFM permission to do so.
