The Washington Academy of Family Physicians thanks you for attending the:

63rd Annual Scientific Assembly
Innovations in Practice

May 11 – 12, 2012
Suncadia Resort, Cle Elum, WA

POST-CONFERENCE RESOURCES

ACCESS/DOWNLOAD THE SCIENTIFIC ASSEMBLY SYLLABUS.  This will be available online until May 25, 2012.  Use the password you received via email before the conference.  If you have lost this password, please email WAFP staff at info@wafp.net and we will resend it to you.

CME Worksheet - use this worksheet to calculate your CME credits.  Credits for physicians are self-reported to AAFP using their website.

Translation to Practice (t2p) - WAFP is participating for the second year in the AAFP Translation to Practice (t2p) pilot program. WAFP/AAFP Member attendees of this meeting have the opportunity to earn two additional Prescribed credits following the CME activity. This program allows learners to translate what they learned in a CME activity into their practice and report back on the results. The learner can choose any one session at Assembly for the t2p activity, commit to implement a change in your practice over the next 3 months, complete the process and earn the additional Prescribed credits.  See the linked document for information and the Commitment to Change form.  Email or fax the completed form to info@wafp.net or 425-747-3109.

ABFM Self-Assessment Modules (SAM) Workgroups: Now that you have completed the Knowledge Assessment Portion of Pain Management, Preventive Care, and/or Coronary Artery Disease SAMs, remember to log on to your account at www.theabfm.org and complete the Clinical Simulation portion. Once the clinical simulation is completed, ABFM will award 12 prescribed credits per module.

LASTLY, SAVE THE DATES FOR THE 2013 ANNUAL SCIENTIFIC ASSEMBLY!

Next year's Scientific Assembly will be held Friday, May 10 - Saturday, May 11, 2013, and the House of Delegates Meeting will be held Thursday, May 9 at Suncadia Resort!  More details to come!
